NeuroLab 360 Scholarship Application
NeuroLab 360 is dedicated to providing highly skilled and specialized care at an affordable cost to all participants in our programs. We recognize that therapy can be expensive, particularly for individuals with neurologic conditions. To help address this challenge, we chose to operate as a non-profit organization, allowing us to fundraise and secure sponsorships to offer scholarships to those who need financial assistance the most.
Our scholarship program provides reduced-cost services, determined by a sliding scale and based on the information provided in this application. Supporting documentation is also required as part of the application process. Your plan of care will be collaboratively developed with your therapist during your initial evaluation and reassessed as needed.
Participation in the scholarship program requires a strong commitment to your treatment program. This includes consistent attendance at all scheduled sessions, adherence to a home exercise program, and compliance with NeuroLab 360’s strict “no-show” and cancellation policy.
No-Show and Cancellation Policy:
The first no-show or late cancellation will not result in any charges.
Subsequent no-shows or cancellations with less than 24 hours' notice will incur a charge at the awarded scholarship rate for the missed appointment.
To ensure accountability and program sustainability, credit card information will be securely collected and retained.
